I approve of the content of this work order. I understand that Kimberly Burke
 dba Golden Needle Quilting will not be held responsible for quilting problems that are a result of piecing problems in the quilt. Your project will be quilted in the condition is is received and will be as free of puckers, pleats, and tucks as your piecing will allow. I will not hold Limberly Burke dba Golden Needle Quilting liable for any damage that might occur to the top or completed quilt while in her possession. I understand that no refunds will be made once the quilt has been completed. If full payment is not received within 30 days of completion of the quilting, or satisfactory arrangements have been made, I understand that my quilt(s) may be disposed of at the quilter's discretion. I also agree that is this quilt is entered into any show, credit will be given to Kimberly Burke for the actual quilting. If said quilt receives recognition or award, I will notify Kimberly Burke of such within one month following the show. I will not hold Kimberly Burke dba Golden Needle Quilting liable for any injuries sustained by me or members of my family while on her premises.
